Straighten Your Teeth with
Interdisciplinary Orthodontics and Periodontics

Over the last few decades, an increasing number of adults who didn’t receive orthodontics as teens have decided to undergo tooth straightening. Adults have more specific objectives and concerns related to facial and dental aesthetics than teens, as well as the type of orthodontic appliance and duration of treatment. Furthermore, adult jaw growth has ceased, so tooth movement through standard orthodontic care is slow.

Traditional orthodontics rely on metallurgy and chemistry by applying force to the crowns of your teeth to move them, ignoring the potential of bone physiology. As such, wearing braces for up to three years may be necessary to achieve correct alignment, which is typically a deterrent for adults with busy lifestyles and careers. At Arcadia Perio, our board-certified periodontist, Dr. Andrew Peterson, works closely with your orthodontist to deliver an innovative method that speeds up orthodontics in Arcadia, CA, with predictable results that are ultimately safer for your gums and jawbone.

A Primer on Accelerated Osteogenic Orthodontics

Accelerated osteogenic orthodontics (AOO), also called periodontal accelerated osteogenic orthodontics (PAOO), is a cutting-edge procedure that combines time-tested traditional orthodontics with advanced periodontal techniques to enhance the health of your gums and bone structure. This orthodontic surgery procedure is based on a natural process called the regional acceleratory phenomenon (RAP) that stimulates bone healing. The two main features of RAP in bone healing are decreased regional bone density and accelerated bone turnover, which are believed to facilitate orthodontic tooth movement.

AOO focuses on how bone responds to orthodontic forces instead of how to apply that force to teeth. A primary focus is to enhance how your periodontium responds to applied forces, resulting in a more intact periodontium and increased alveolar volume to support teeth and overlying gum tissue. While many patients are suitable candidates for AOO, contraindications include bone loss, root damage, active periodontal disease, Class-III malocclusion, and rheumatoid arthritis.

What to Expect When You Get Orthodontic Jaw Surgery with the AAO Approach

The first step is a thorough orthodontic and periodontal evaluation with Dr. Peterson in collaboration with your orthodontist. For the treatment to be successful, no active disease should be present during the surgery; therefore, gum disease treatment is required before the start of surgical orthodontics in Arcadia, CA. Dr. Peterson will explain everything you need to know, including the procedure’s limitations, potential issues with the outcome, and the schedule for regular monitoring visits.

Cone-beam computed tomography scanner and 3D imaging enable Dr. Peterson to maximize surgical precision without soft tissue injury, enabling rapid tooth movement with orthodontic surgery. The AAO procedure involves creating a flap to provide proper access to the alveolar bone, decortication, particulate bone grafting, closure of the flap, and orthodontic force application. Under sedation and a local anesthetic, Dr. Peterson creates the flap, selectively removes cortical alveolar bone (dense, bony plate around certain teeth), establishes a bone graft in this area, then closes the flap with sutures. Any significant movement requiring orthodontic force is not performed until at least two weeks after surgery to allow RAP to take effect.
